Organisational review questionnaires - detailed instructions for purchases

The following four on-line surveys have been designed to provide feedback to your organisation. They can also be used as paper surveys.

A. Organisational Review - To what extent does your organisation have the characteristics of a thriving organisation? This could be completed by managers, staff or other people who are very familiar with the organisation.

B. Board Review - To what extent is your Board working well and governing well? This is for board members to review their own work .

C. Staff feedback - How do your staff experience working for your organisation? This is for staff to complete.

D. Stakeholder feedback - How do other services and stakeholders perceive you? This is for other other stakeholders and services to complete.

You are welcome to browse the surveys

If you wish to use any of these four surveys in your organisation there are two options.

1. Edit and use any of the four surveys as you wish within your organisation

a) BUY a copy of the Organisational Review Starter Pack for $25. It is a Word document Wordthat includes text and layout for the four surveys.
b) Modify the document to suit your needs.
c) When you reproduce the survey acknowledge the copyright source (Paul Bullen at www.mapl.com.au 2008).
d) Use and modify the survey as much as you wish in your organisation. You do not have permission to provide it to other organisations.

2. Use on-line versions of the surveys with the ability to modify them to better suit your needs

a) OPEN a SurveyMonkey account at Survey Monkey - this will enable you to run your own on-line surveys. There is no charge to open an account (but survey length on the free accounts are limited to 10 questions).
b) After your account is opened upgrade it to either a monthly PRO or annual PRO. Current pricing is approximately US$20 per month. You only need to pay a subscription for the months where you are actively using on-line surveys.
c) Request that your Survey Monkey account can use HTML codes in the survey. To do this Login to your account. Click on the Help Centre. When in the Help Centre click on Contact Support. Make a request: Please enable HTML codes in the surveys in my account.
d) Wait for confirmation that your account has been activated to use HTML codes (usually within 4 hours). If you do not do this the organisational review surveys you purchase will show all the HTMML codes through the text of the survey - not a pretty sight.
e) BUY the Organisational Review On-line Surveys for SurveyMonkey for $25 (for all four organisational review surveys) from this site.
f) Notify us of your Survey Monkey account name in the purchase process
g) The on-line version of the four surveys will be uploaded to your Survey Monkey account within 24 hours of your purchase.
h) You can then modify the surveys in anyway you wish and have full control over all aspects of the on-line survey process.
