Sample organisational review questionnaires - copyright fees

If you use any of the following surveys in your organisation please pay the appropriate copyright fee.

     A. Organisational Review
     B. Board Review
    C. Staff feedback
    D. Stakeholder feedback

The fees for Non-profit organisations using the sample organisational review questionnaires as a basis for their own surveys must pay a copyright licence fee of $10.00 for each sample survey used. (e.g. if your organisation uses the staff feedback survey as a basis for developing your own staff survey the copyright fee owing is $10.00. If you then use the sample Board Review survey to develop your own Board survey a further copyright fee of $10 is owing.

The fees for For profit organisations using the sample organisational review questionnaires as a basis for their own surveys must pay a copyright licence fee of $25.00 for each sample survey used. (e.g. if your organisation uses the staff feedback survey as a basis for developing your own staff survey the copyright fee owing is $25.00
